- Obtain all contact information from potential presenter including name, address, phone number, and preferred mode of contact to receive presenter packet (via e-mail, fax or mail) or have presenter access application and AOTA Classification Code Sheet at nysota.org.

- NYSOTA will process information and send presenter packet via e-mail, fax or mail.
Potential presenter receives presenter packet, completes it and mails or faxes the application and all necessary information required to: NYSOTA, 119 Washington Avenue, 2nd Floor, Albany, NY 12210 518-432-5902 (fax) - When received by NYSOTA, the packet and materials are sent to the CE Chair and reviewed for completeness. CE Chair will solicit other individuals as indicated and appropriate for assistance in reviewing applications for approval.
If complete and meets requirements, presenter is approved and an Approval Letter is sent to presenter by NYSOTA (information is added to database). - If information is not received or is incomplete, CE Chair will follow up with potential presenter to determine confirmation of receipt, to request outstanding information or provide clarification. CE Chair will cc District Chair of interest and others as indicated.
- All completed paperwork will be forwarded to NYSOTA for filing for audit purposes.
- Preparation for workshop at the district level; create the brochure (include objectives, AOTA Provider Approval statement and NYSOTA Disclaimer) and forward it to the State CE Chair for approval; acquire paperwork (i.e. Sign-in/out Sheet; Course Evaluation Forms, Instructor Self-Evaluation Form, AOTA on-line survey handout), set up registration table, designate a room monitor to introduce speaker; get signatures/completed paperwork and provide copy of Day of Certificates. Give participant AOTA on-line survey handout-needed for application process.
- Day after tasks: Send to NYSOTA the Sign-in/out Sheets, Course Evaluation Forms, Instructor Self- Evaluation Form and a check made out to NYSOTA (with CE written in memo section) for 50% of the proceeds from session with a minimum profit of $200. ($100. for District and $100. for CE).
** This process should be efficient and done in a timely manner as districts want to get all parties involved, and should be cc’d on all correspondence. **
